Welcome to the AMITA Health Care Network (AHCN) Member Portal. AMITA Health Care Network is a Population Health Team supporting AdventHealth and Ascension Illinois. If your provider is affiliated with one of these hospital organizations, you may receive reminders from us to help keep you up to date with your preventative care. The Member Portal features a provider lookup tool as well as content related to the network. It is our hope that this will assist you.
