AMITA Health Care Coordination Program


AMITA Health Care Network's (AHCN) Care Coordination Program provides complex case management for those with complex needs. The program also provides case management for those with short-term needs, as well as condition management support for those with asthma and diabetes. Care Coordinators are members of the Ascension clinical care team who work closely with doctors to develop customized approaches for each patient's specific healthcare needs.

What is an AMITA Health Care Coordinator?

Your AMITA Health Care Network Care Coordinator (AHCN) is a member of the Ascension clinical care team who works closely with your doctor to develop a customized approach for your specific healthcare needs.

How will working with an AHCN benefit me?

Your Care Coordinator will partner with you to help you reach or maintain your health and wellness goals.

While in the AMITA Health Care Coordination Program you will:
  • Be able to discuss your treatment/care and get answers to your health questions.
  • Learn to monitor your health habits and take any needed steps for change.
  • Work with your Care Coordinator to develop personalized healthcare goals that will be shared with your doctor, helping you and your doctor to become active partners in your healthcare.
  • Receive useful health education materials and assistance in finding community resources.
  • Have your own Care Coordinator who will call you regularly to offer support between your doctor visits, which is often when you need help the most.
  • Get coaching from your Care Coordinator on ways in which you can easily care for your health condition and make lifestyle changes to progress toward your health goals.
  • Have your Care Coordinator’s contact information, so you can reach her or him.

What will my role be?

By working with an AMITA Health Care Coordinator, you agree to maintain open communication with your Care Coordinator and your healthcare team and to participate in your care planning, health goal development, and medical decisions.

Will the Care Coordinator have access to my medical records?

Yes. Your Care Coordinator will have access to your medical records and will work as part of a team with you and your doctor so that everyone agrees on the same health and wellness goals for you.

What if I have questions regarding a specific medical condition or treatment?

The AMITA Health Care Coordination Program does not replace the care of a doctor. You should share any questions or concerns you have with your doctor.

Your Care Coordinator is part of your healthcare team who works with you and your doctor, so you have an additional resource to help you reach your best health.

Is there any cost for the AMITA Health Care Coordination Program?

The program is offered at no cost to you or your insurance. Your health insurance benefits will not change if you do or do not decide to take advantage of the program.
